Enrollment Procedure
  1. Read ALL of the materials in your enrollment packet. (See forms below.)  
       
  2. Complete the New Student Application.  
       
  3. Return to the School Office, as soon as possible, the following documents:  
   
  • Completed New Student Application for each new student
  • Non-Refundable Registration and Special Field Trip Fees (see below)
  • A copy of your child’s birth certificate
  • A copy of your child’s Social Security card
  • Signed Partnership Agreement
  • Completed Fee Remittance Agreement form
  • A copy of your child’s most recent report card, achievement testing, IEP or other testing results.
  • Kindergarten Readiness Survey for Kindergarten students
  • Administrative Recommendation and Classroom Teacher Recommendation
  • Florida Department of Health Form 680
  • Florida Physical exam form
  • Custody/Guardianship/Adoption (if applicable) paperwork
 
       
    Class size is limited, so spaces will fill quickly.  Enrollment is determined after review of the application, report card and the screening assessment testing.  
       
  4. An enrollment assessment for readiness and success will be arranged by the School. You will be contacted by the School Office to set up a testing time.  A charge of $50.00 is due at the time of testing.  Once the screening results are available, you will be notified and your child will be placed on the enrolled student list.

Refer to the Immunization and Record Requirements sheet enclosed.
Arrange the necessary doctor’s appointments, etc. to comply with the required Health Department immunizations and documentation.All requirements must be completed and proof submitted to the School Office before the first day of school

 
       
  6. Fifth Grade:  $100 deposit for Sea Camp trip;  Eighth Grade:  $100 deposit for Washington DC trip;  Additional fees for the 5th and 8th grade field trips will apply.  Information will be made available after school begins.  
       
    *You can become a member of St. Luke’s by: baptism in the name of the Father, Son, and Holy Spirit, for infants and children who have never been baptized; transfer or reaffirmation of faith, for those who have been confirmed in a Lutheran Church; attending instruction classes for adults who have not been baptized or confirmed in a Lutheran Church.  Membership presupposes the desire for regular (every week) worship participation.
